Medication at School

     If you need to send medication to school, it must be kept in the school clinic and a Medication Authorization Form should be on file. Please use the links below to access the form. The form must be filled out completely and brought to school with the medication. Parents must deliver the medication to school.

     Students with allergies and/or asthma should have an Asthma/Allergies Emergency Card on file. Please use the link below to access the forms. Print them out, complete both pages and send the completed forms in to the school.

     Athletes who are trying out for sports teams must have a sports physical dated after May 1, 2009 on file before you will be permitted to try out. Forms may be downloaded from the link below or picked up in the school office.
